Check in notes
It's essential for our care coordinators to document regular updates foreach individual under our care. Please ensure a check in note is recorded every two months for every care recipient.

Defining a check in note:
- A check in note encapsulates a client's ongoing development and status at specific intervals.
- Every two months, a check in note summarising the evaluation of the client's progress must be submitted.
- These entries should detail the client's journey toward their objectives, highlight any notable changes or advancements, and include any input the client may wish to offer.
- Utilising check in notes, we can track the client's trajectory, spotlight areas for enhancement or concern, and enhance the collaborative communication within the care team.
In the portal, a check in note is determined by the note category.
Package list
On the package list table, there is a column title 'Check Ins'. This column indicates if there are any items that need actioning.
The check in note status is represent by a coloured note icon in the Check Ins column.
- Red Icon: Indicates an overdue check in note.
- Orange Icon: Signals an impending due date within the next month.
- Green Icon: All check in note submissions are current.
- Black Icon: A check in note is missing for the package.
You can filter the package by check in note status.
You can view additional information on the check in note by clicking on the note icon.
Notifications
Care coordinators will receive email notifications if there is an upcoming check in note due, a missing check in note or an overdue check in note.
Check in notes are required to be written every two months. An upcoming check in note alert will be triggered one week prior to the check in note due date. Once the due date passes, the check in note becomes overdue.
Daily notifications will continue to be sent as a reminder to create the check in note.
