Add a Home Care Package contact
If you are the recipient or the primary representative, then you will be able to add new contacts.
On the package details page, navigate to the Package Contacts section.
Click on the 'Add new contact' button to begin adding the contact information.
Fill in the required fields on the Add package contact screen. These are the name, email and phone number. You will also have to select the relationship and authority type of the contact.
- The relationship is how the contact is related to the recipient.
- The authority is what authority the contact has in relation to the Home Care Package. There are three options to choose from.
- Day to day decisions - can make decisions on day to day management and coordination of the Home Care Package
- Emergency decision making - someone that can assist in an emergency
- Follower - all other contact types
- We also have Medical authority and Financial authority options, but these are only added with Enduring Power of Attorney (EPOA) and added by Trilogy Care staff.
- There are also two additional check boxes to select.
- Emergency contact
- Lives close to the recipient
Once the form has been completed, click 'Save contact' and you will be redirected back to the package details page.
Once a contact has been created, you can invite them to view the Home Care Package on the portal.
